Implant and Periodontal Plastic
Surgery in the Esthetic Zone

| We live in a culture that judges its
members by appearances. To us,
a smile echoes health, wealth, demeanor
and worth. Any departure from the ideal
— missing front teeth or altered gingival
contours especially — devalues
a person.
This course trains you to perfect
the dentition with periodontal
plastic surgery and to restore anterior
edentulous areas with implants
indistinguishable from natural teeth.
We provide a step-by-step tutorial
that enables you to diagnose, design and
refine soft tissue and implant cosmetics
for your own patients. Live surgery and
a hands-on practicum shall cement
your knowledge. In particular, we will
showcase the newest as well as most
predictable grafting methods known
to attain a harmonious smile. |

Redefining Perioplastic Surgery
“Thinking Outside the Palate©”
Most perioplastic surgery for grafting can now be done without palate surgery
for free gingival grafts or connective tissue grafts. Acellular dermis can be used
in a simplified technique without the necessity of tedious and time consuming
tunneling.
Placental membrane is a new product that brings more options to treating
recession defects due to lost attachment. It is easily placed, does not require
suturing, and heals without infection or inflammation.
Course Objectives for Participants:
- Recognize the prevalence of recession defects in our patient population.
- Describe gingival soft tissue anatomy, blood supply and wound healing.
- Utilize soft tissue manipulation with incisions and flap design for predictable graft results.
- Understand the contributory factors in root coverage grafting.
- Understand the value and use of biological growth factors in soft tissue surgery.
- Describe the indications for gingival grafts, connective tissue, acellular dermis, and palatal derived membranes for attachment gain, root coverage and implant augmentation.
- Diagnose and treat recession and lost attachment sites with soft tissue surgery.
- Predict and prevent soft tissue grafting complications.

Surgical Program
S1 Patient Evaluation & Treatment Planning
S2 Root Form Surgery Division A Bone
S3 Membrane Grafting Division B Bone
S4 Sinus Grafts
The Misch International Implant Institute™ was developed in 1984 to help set and elevate the standard of
care in implant dentistry using a hands-on approach. Now world renown, the Institute’s goal is to remain
at the forefront of implant dentistry through research, education and its unique clinical applications. Using
these tools and a well-trained faculty, the Institute is able to provide its students with the most progressive
and documented information.
Carl E. Misch, BS, DDS, MDS, PhD (hc), FACD, FICD
Carl E. Misch is Clinical Professor and Director of Dental Implants, Kornberg School of
Dental Medicine, Temple University. He is co-chairman of the International Congress of
Oral Implantology (ICOI), the worlds largest implant organization. He has written four
textbooks on Implant dentistry, both on surgery and prosthetics, which have become
the most popular books in all of dentistry. He was the Director of the Oral Implantology
Residency Program at the University of Pittsburgh School of Dental Medicine from 1989
to 1996. Dr. Misch has maintained a private practice restricted to implant surgery (bone
grafting and implant placement) and related prosthetics for more than 25 years.
The Surgical Program was developed by Dr. Carl Misch for practitioners whose interest in is the surgical phase
of implant dentistry. This five session course emphasizes the differences between bone divisions and the
different nuances of each type of surgery. Participants will attend specific lectures on treatment planning
before moving to root form surgery and membrane grafting. These course are followed-up by advanced
techniques such as sinus grafting and immediate load surgery. Dr. Misch’s book, Contemporary Implant
Dentistry, 3rd Edition, will be provided at Session 1 and handouts at each session relating to the covered topics.
Surgical Program
| S1 Patient Evaluation & Treatment Planning |
 |
|
The first session’s major objective is treatment planning for the partially edentulous patient. Prosthetic
options, patient force factors, bone density, implant positions, number of implant abutments, ideal implant
size for each tooth location, and bone volume related treatment options are presented in detail. CT and
radiographic interpretation workshops and hands-on suturing techniques and implant insertion in the
treatment models are also provided. The surgical procedure and prosthesis type presented in the first session
is the replacement of a mandibular single posterior missing tooth.
|
| S2 Root Form Surgery for Division A Bone |
 |
|
The second session of the Misch International Implant Institute™ advances the clinician’s skill level and
knowledge of root forms. The focus of this session will include surgery, uncovery and short and long term
complications for root forms. Complications presented include: excessive bleeding during surgery, nerve
damage and bone loss during initial healing. The second surgical session addresses partial and completely
edentulous mandibular overdentures and fixed prostheses for multiple adjacent missing teeth in the
posterior mandible. Altered surgical approaches, dependent upon bone density are presented in detail. The bone volume classification covered at
this session is Misch-Judy Division A and B. Hands-on labs will give clinicians the chance to apply learned techniques in a controlled environment.
|
| S3 Membrane Bone Grafting for Division B Bone |
 |
|
The third session of the MIII™ is primarily concerned with two implant issues: Maxillary anterior single tooth
implants and barrier membrane particulate bone grafts. The volume of bone and related procedures covered
in the session is for Division B bone augmentation using a “layered approach” as developed by Dr. Misch.
Atraumatic extractions and socket grafting are also presented. Indications for extraction and immediate
implant insertion, esthetic complications of implant position and angulation are also covered in detail.
|
| S4 Sinus Grafts |
 |
|
The fourth session is dedicated to treatment of the posterior maxilla. Sinus lifts and sinus grafting are
presented in detail. The unique aspects of the program include maxillary sinus pathology and pharmacology.
Complications of this region are also covered in detail. The hands-on laboratory sessions are performed on
unique models developed specifically for the MIII™ for both sinus lifts and grafts.
